What to bring to your First Appointment
All new patients are required to bring the following to their first appointment:
- All medications you are currently taking in their original containers
- Any recent MRI, CT Scan or Xray reports
- Driver’s License
- Insurance Card
Check-in From Home!
The Augusta Pain Center has implemented PHREESIA PRE-VISIT which allows you to fill out your paperwork from the comfort of your own home. It will save you time and help expedite your upcoming office visits. All we need from you is a valid email address. This new feature is fully encrypted and fully integrated with our Electronic Health Records System. All your information is in the system when you arrive!
You will receive an email and instructions a few days prior to your scheduled appointment date from apcpaperwork@gmail.com. Please follow the instructions for security purposes and then submit your New Patient Questionnaire. If you are unable to complete your New Patient Questionnaire from home, you will required to arrive 45 minutes prior to your appointment to complete your New Patient Questionnaire on a Phreesia tablet in our reception area. If you do not arrive 45 minutes prior to your initial visit, your appointment will be rescheduled for the next available opening.
